Start Date: June 30, 2025
Shift: Monday to Friday, 8-hour days
Guaranteed Hours: 40 hours per week
On-call/Weekend Requirements: Ideally yes, but flexible arrangements possible
EMR System: HCHB
Estimated Weekly Salary: $1350 - $3180
Job Requirements
Minimum 1 year of experience in home hospice care
Current Registered Nurse (RN) license
Strong communication and organizational skills
Ability to work independently and as part of a healthcare team
Responsibilities
Provide comprehensive case management for hospice patients
Coordinate care plans and collaborate with interdisciplinary healthcare teams
Conduct patient assessments and develop individualized care plans
Offer emotional support and guidance to patients and families
