Planet Healthcare is looking for a Hospice Social Worker in the Decatur, IL area.
Shift: 8 hours, Days Shift, 40 hours per week

Job Description & Requirements:
- Assignment Length: 12 weeks
- Start Date: ASAP Pending compliance
- Employment Type: Contract
- Previous Hospice experience - Highly Preferred but not Required
- Masters in Social Work - Required
- License of Social Work for state of IL - Required
- Additional Mileage Reimbursement provided while on assignment for home visits
